Men’s Health: When “normal” labs don’t explain how you feel

Here’s a pattern I see frequently. A man feels different. Energy is lower. Workouts don’t hit the same. Motivation feels flat. Sleep is no longer restorative. He gets labs with his PCP and is told, “Everything looks normal.” But he still doesn’t feel like himself.

That disconnect is what I call the “normal labs” trap.

Most lab reference ranges are statistical averages. They tell us what is common, not necessarily what is optimal for how a man wants to perform, think and feel. In men’s health, one number rarely tells the whole story.

Total testosterone measures everything circulating in the blood, but much of it is bound to SHBG (sex hormone binding globulin). Bound testosterone is not readily available to tissues. Free testosterone, the active portion, is what drives energy, strength and drive. A man can have a “normal” total testosterone and still have low free testosterone. If only one number is checked, that nuance is missed.

Thyroid imbalance is another issue I see often, even in men who were told their thyroid is “fine.” The standard screening test, TSH, does not always tell the full story. Looking at free T4, free T3, rT3, iron status and clinical symptoms can reveal patterns that explain fatigue, brain fog, weight resistance or low mood. Iron is especially important. Low iron can impair thyroid conversion and oxygen delivery to tissues, leading to sluggish energy even when other labs appear normal. If thyroid signaling is off, testosterone alone will not correct how a man feels.

Sleep and stress also matter more than most realize. Poor sleep, untreated sleep apnea and chronic stress elevate cortisol, which can blunt hormone production and alter how the body uses testosterone. When these drivers are not addressed, men may continue to feel off despite “normal” labs.

The goal is not to chase lab values. It is to understand the full physiology and interpret it in context. If you have accepted “normal” but do not feel normal, it may be time to look beyond a single lab value. A comprehensive assessment can provide clarity and, in many cases, meaningful improvement.
